FDOH Nassau County Health Department has received an initial supply of Moderna COVID19 vaccine. If your dialysis/home health/hospice/visiting nurses organization is interested in receiving a limited supply of COVID19 vaccine to administer to your healthcare workers with direct patient contact, please complete the attached Vaccine Site Agreement and the Phase 1a Exhibit document and fax them to Sherry Linback at 904.428.5634 as soon as possible.
Due to limited supply, we cannot guarantee the Health Department will provide the amount of vaccine requested but we should be able to transfer an initial supply (as well as the second dose in 4 weeks.) Participating in this effort will require an organizational designee to receive information/training on vaccine storage, handling and reporting requirements when they pick up the vaccine at the Yulee Clinic. Please note, this vaccine must be used in accordance with the Governor’s Executive Order 20-315 and only provided to the current listed priority groups:
- Long-term care facility resident and staff
- Persons 65 years of age and older; and
- Health care personnel with direct patient contact.
We have chosen to prioritize organizations that are likely to care for Nassau homebound, underserved or medically vulnerable older adults. If you do not receive enough vaccine for all your direct care staff, we ask that you prioritize licensed staff who are able to vaccinate both additional staff and in the future, also vaccinate your clients. For your planning consideration, the vaccine comes in 10-dose vials and the doses must be administered within 6 hours of opening the vial. If you are uncertain that you can use all 10 doses at one time, you are encouraged to partner with another like Nassau organization to allow rolling staff vaccination times.
